Lower Back Pain (Lumbago)

Lumbago refers to acute pain in the lower back area (commonly known as a “sudden back pain” or “lumbago”), regardless of its cause or how it presents. It is often confused with sciatica. The main causes of lumbago include excessive strain on the lower back, herniated discs, vertebral displacement, or congenital spinal anomalies. A sudden sharp pain that can occur when bending or lifting something heavy is usually caused by the displacement of an intervertebral disc or severe tension in the back muscles and ligaments.

If lumbago is accompanied by sciatica, it is most likely due to a displaced intervertebral disc.
In rare cases, rheumatic diseases, certain infections, or tumors may cause lumbago. The painful sensation of lumbago occurs due to irritation of receptors located in the annulus fibrosus of the affected disc or the adjacent ligaments. This irritation causes tonic muscle tension in the area. However, even with modern diagnostic methods, the cause of lumbago remains unknown in 30% of cases.
Lumbago is most common in men between the ages of 30 and 40.

Symptoms:
Typically, lumbago is triggered during or after intense physical activity, overheating (with sweating), followed by cooling. The sudden and sharp pain in the lower back is described as tearing, throbbing, stabbing, or burning. Subjectively, the pain feels deep in the muscles, ligaments, or bones.
During an episode, the person may feel helpless, frozen in a forced posture, as any movement attempt increases the pain, and muscle spasms develop in the lumbar region. The duration of the pain can range from a few minutes to several hours or even days. The pain tends to decrease when lying still on a firm surface. Interestingly, the sharp pain may disappear as suddenly as it appeared, leaving patients with a feeling that "something popped back into place."
In cases of severe pain, functional methods such as pain relievers and antispasmodic medications are used.
