The Family Card is a shared card used by family members and registered under the family surname.
Bonus System
Family members who own a Medical Card may receive a combined bonus, which is credited to the Family Card.
Benefits
- Depending on the annual spending, 5-20% of the amount paid by each family member is credited as a bonus to the Family Card.
Terms & Conditions
- The card cannot be used by individuals not registered under the Family Card.
- To activate bonuses, a family member must present an identity document along with their individual Medical Card.
- Accumulated bonuses can be used for check-ups and other medical examinations.
- Patients can partially pay for services using bonuses, with the remaining balance covered by an additional payment.
- If the bonus amount exceeds the cost of a service, the remaining balance stays on the card and can be used for future medical services.
- Bonuses credited to the Family Card can be used by any registered family member upon presenting an identity document.
- Accumulated bonuses can be used all at once or in parts.
- In case of card loss, a family member with an identity document may request a replacement at Saglam Ailə Medical Center.
- A new card will be issued for 5 AZN, maintaining the previous medical history and bonus balance.
- The right to modify the Family Card system belongs to the Saglam Ailə network of medical centers.
